Honored, Sir.

I make bold to Address you. In Regard to My self. I have been in the Servis over three years I Belong to the 34 ill. u.s. Regt. Am Not tired of the Servis, in the field, but [hartly?] tired of Hospital life, Have been In the Hospital since the first of May last have been At this Hospital since the 9 of July. have Neve been Examined yet. I would like to join my Regt the doctor says that I cant stand field duty Any More. I would like to have My discharge & if you will Assist me. In getting. said discharge. I shall be vary thankful. yours with Respect Hiram N Clark

My Diseas is inflamation of the kidneys. pleas write to me & let me known what you will do. for A soldier that has done his duty. while at the front
