To the Surgeon of any Hospital

to whom this may be presented.

Dear Sir

The bearer of this, Dr. J. C. H. Hobbs of the 12th Iowa Infty, has been with our Regt since and before it was mustered into service.

During the whole time he has been faithfully at his post, ever active in the discharge of his duty, exposed to toils and exposures till he is almost worn out. It is impossible to get a sick furlough here for any one, as it is against the positive orders of the Commanding general.

In consideration of these facts we would respectfully and earnestly ask, that you grant him a sick furlough immediately to go to his house in Illinois.
