Dear Sir

I wish to visit the State of California for my health and want to know if you can give me a permit for that purpose.

I have been for several years seriously troubled with an affliction of the throat and lungs, with at times a Hamorhage of blood from the lungs. This I can verify by a certificate from my Physician corroborated by a certificate from the Mayor of the City also the County Clerk & others if necessary
