Sir

I have the honor to acknowledge the receipt of your not of the 5th inst. and beg leave to submit the following statement.

Noah Guyman, of the 31st Ill Infantry, who was for a long time a nurse in this Hospital, applied to Med Inspector Col. Allen to be discharged from service. no sufficient cause being apparent it was refused, he has since then returned to his regiment
